For Unemployment Insurance Tax Only, Quarterly and Annual (Domestic)
File Form 132 if you have payroll subject to unemployment insurance tax (ORS. Chapter 657). This includes reimbursing and Local Government Employers Benefit Trust Fund (LGEBTF) employers. This form is not required if there is no payroll to report for the quarter.
The Form 132 contains the social security numbers (SSNs), names, hours, wages for each employee, State Withholding (Whole Numbers only), and employee reference numbers (optional). The columns can be sorted by clicking on the title.
